Visit information
Initial visit
Please bring previous x-rays and reports with you, if you have them, for your initial visit. Also, wear loose fitting clothing if possible. The initial visit lasts about 1 hour and future treatments take about 15 minutes to 30 minutes.
Please also print and complete the two Patient Entrance Forms (PDF) and bring them with you the first time I see you. This will allow you to fill out these forms at your leisure.
